First Impression : DNB Gastroenterology at Apollo Glengales hospital, Kolkatta

Edited By Dr Gajanan Rogde

I was a bit hesitant initially to leave Mumbai after 10 years of MBBS and DNB Medicine. But now after one and a half years, i must say opting for DNB Gastroenterology at Apollo Gleneagles Hospital was one of my best decisions. I will be giving a brief overview of DNB Gastro at Apollo Gleneagles Hospital.Apollo Gleneagles Hospital Kolkata, a 700-bedded multi-specialty tertiary care hospital, is a great combination of technological excellence, complete infrastructure and competent care. It is the only hospital in Eastern India to be accredited with the Joint Commission International (JCI) accreditation, the international benchmark for quality. Institute of Gastrosciences and Liver is one of the top gastroenterology institutes in India and one of the very few hospitals which has a separate gastro intensive unit. It has launched India's first Cellvizio System.

Our Director and HOD, Dr.Mahesh Goenka sir is a world renowned gastroenterologist and a perfect blend of an extraordinary clinician, enthusiastic academician and a skilful endoscopist. Around 5 to 6 post DM/ DNB consultants, 2 intensivists, 6 DNB Registrars and 10 to 12 RMO’s form the gastroenterology team. On an average 60 to 80 patients are admitted under Dr.Goenka sir in male ward, female ward and gastro intensive care unit.
During the three years tenure of DNB, you shall be rotated in ward, gastro ICU, OPD and peripheral postings (Radiology/Pathology). Endoscopy procedures will be daily in the evening after 4.00 pm. The duty hours vary upon where you are posted. During ward posting, you will be posted either in male or female ward under one of the post DM/DNB consultants. The DNB trainee will be completely incharge of that ward and will present rounds to the consultants - first the post DM/DNB consultant and then with Dr.Goenka sir. One post MBBS RMO will be there to assist in ward work and night duties. Usually in the ward, you start your day at 8 to 8.30 am and finishes by 8 to 9.00 pm in the evening. On an average 20 to 25 patients are admitted at a time in male or female ward. For the OPD posting you are given a separate cabin (by your name) where
you have to see patients allotted to you. Around 70 to 90 patients visit the OPD daily and are distributed among 6 to 7 doctors. After the patients are seen by the respective doctors, they go to Dr.Goenka sir with the case sheet where sir gives the final advise. The case sheet then comes back to respective doctors for final signature where you get to know the changes (a good learning experience where you come to know the clinical findings you have missed and mistakes if any).
In the ICU all critical GI patients like GI bleed, Acute Pancreatitis, CLD with HE and others are admitted. During the ICU posting average duty hours are 60 hours/week.
By the end of first year, you start getting Upper GI endoscopies, by the end of second year colonoscopies start and by the end of final year you get therapeutic procedures. On an average 200 endoscopy procedures and 40 ERCP’s are done per week. By the end of three years, you will be well trained in Upper and Lower GI endoscopy procedures.
Coming to the most important part i.e academics. Every Tuesday and Friday seminars are scheduled from 9 am to 10 am where the whole department gathers. The initial 40 min presentation is by DNB Trainee and other 20 min part is by one of the consultants. The academics take place sharp at time and without fail. My third year senior has passed both theory and practical in the first attempt (only one DNB seat was there previously, now it has increased).
Depending on the situation you get around one and a half months of preparatory leave for your exams. Stipend is 75,000 Rs per month through first year, second year and third year.Unfortunately, the hospital does not provide accommodation, but you usually find a good place to rent nearby the hospital. Post DM/DNB candidates from all over India come to learn advanced endoscopy procedures like ERCP and EUS (1 to 2 year duration) at our institute.
Finally APDW, the biggest GI meet of Asia is being hosted by our department in December 2019, where Dr.Goenka sir is the president.
I personally chose Apollo Gleneagles Hospital because :
1. Get to work under one of the best Gastroenterologist Dr.Goenka sir
2. Good hands on for UGI and LGI endoscopies
3. Regular academics and teaching
4. Exposure to advanced endoscopic procedure
