In focus : DNB General surgery at Voluntary Health Services, Chennai

The Voluntary Health Services is a 465 bedded trust run hospital located in Chennai. It has completed 50 years of non profitable health care services. All general specialties and a few super specialties including a blood bank, haemophilia centre and de-addiction programme are available. ICU, Dialysis, laboratory investigations etc are available at a nominal cost. About 70 % of the patients (those with a monthly income below a certain limit) are treated free of cost. How many consultants are there in your department?

There are 3 units in general surgery. 2 consultants in each unit.Apart from this one consultant each for plastic surgery, surgical gastroenterology and urology

What are the usual working hours??

Starts at 8am. Depending on the day it can extend till 3pm or 5pm. After that evening rounds for an hour at 8pm.

How many emergency duties per week /month?

Usually there are 3 to 4 duties per month for each pg. OTher days for any surgical emergencies first year PG has to go and attend the case first. Trauma cases are quite less so that doesn't disturb sleep much. Monday to Saturday morning 10 to 1 is the usual op timings. Normal census for General Surgery op is around 30 to 35 cases per day.Op duty includes I&D, follow up dressing, diabetic foOT debridement and working up new cases for surgery After finishing the OPD first year pg can join the seniors in OT. First year pg will have one OT day where he will be exclusively in OT. Second and third year pgs get 2 OT days a week. Apart from that second year pg will attend urology OT on Saturdays. Third year pg is given preference for surgical gastro and all major procedures.Gen Surgery theatre days are Monday Tuesday and Thursday. 10 to 12 cases in general surgery are posted per week. Emergencies and local cases are nOT included in this list. Friday is plastic surgery OT day and 3 to 4 plastic surgery op also will be seen by the consultant. Saturday is urology OT day and after OT 8 to 12 op cases for urology will be seen

Surgical gastroenterologist consultant visits on a honourary basis who does 2 cases per month (like hemicolectomy, gastrectomy and other heroic procedures) for which General Surgery pgs have to assist.Sunday there is no OPD

If its a surgical branch then what all surgeries you have performed till now?

All minor OT cases i am allowed to do individually that includes lipoma, sebaceous cyst etc. And after 3 months I have gOT my chances in bottom cases (haemorrhoidectomy, fistula, fissure), hydrocele. Also i was lucky to do one SSG. Hernia i have did from mesh fixation to closure. For OTher major cases like open chole I have been third assistant. Chances keep increasing as the period of training goes on.

Some super speciality cases that my seniors have assisted:

Pyeloplasty

Hemicolectomy

Hartman's

Pancreatic necrosectomy

Stapler anastamosis (in GE cases)

D2 gastrectomy

Contracture release

Accessory breast excision

Are you allowed to touch the patient??

No doubt. Pgs see the patient first. Present to consultant.

How are the academics? Any case presentations done?

There is no schedule for case presentation. Before my super senior (who has written a blog about the same institute on freeassociation) appeared for practicals we had 10 to 12 case presentations that were par excellent. Other than that any cme programme within the city we can attend anytime. Other national conferences second and third year pgs usually attend.

How is the pass rate?? have your seniors passed?

My super senior cleared both theory and pracs in first shot. Previous batch people have all cleared till now though few did not clear practical’s in first attempt.

How satisfied are you with your choice?

Super Satisfied!!

Any Other departments at your institution good for DNB??

Neurosurgery and OBG are the other departments. Cutting chances are quite less in OBG. Neurosurgery is a very disciplined department. NOT sure about their cutting chances.

Stipend  

First year 25k

Second year 27k

Third year 29k

Accommodation is provided within the hospital premises that don’t have any faults.

only one pg per year in general surgery. So the no of chances is very satisfying. Appendix and chole are usually done in laparoscopy. And in final year you get very good lap chances. Also you get chances to do endoscopy individually with consultant beside for help. It's a nice learning experience to assist super speciality cases with the consultant which one can't get as a general surgery pg. Also we get good cases in cm health insurance scheme. Since medical oncology department is also attached we get few cases that are operable post chemo or radiotherapy.

And you are in one of the best cities with beaches nearby ! Having no co pg makes you have no full time companion. That's the only drawback i could feel.

Why did you chose this hospital?

General surgery was my field of interest

Almost equal to a govt setup in terms of cutting chances but with less clerical works

Since it's my home city where i did my schooling and UG i was sure about the place before taking my seat.
